Under normal conditions, relatively few bacteria live in the small intestine. Food moves through, nutrients are absorbed, and the remaining material continues into the large intestine, where most of your microbiome resides.

In SIBO, bacteria begin colonizing the small intestine in excessive numbers. They start fermenting carbohydrates far earlier than intended, and that fermentation produces gases.

Those gases become one of the clues we measure during breath testing.

Many digestive problems produce symptoms hours after eating. SIBO often behaves differently. Because bacteria are already waiting in the small intestine, fermentation can begin almost immediately after food arrives.

Many people notice bloating soon after meals, along with excess gas, abdominal distention, pressure, belching, and changes in bowel habits.

The timing doesn’t diagnose SIBO. It helps decide whether a breath test is worth doing.

The measurement

A breath test, not a blood draw.

After drinking a specific sugar solution, breath samples are collected over several hours. If bacteria are fermenting that sugar in the small intestine, they produce gases that pass into the bloodstream and are eventually exhaled through the lungs.

Different gases point in different directions, so we watch how they rise and fall across the test.

The goal is not simply identifying a gas. It is understanding the pattern of gas production over time.

Treating bacteria doesn’t automatically solve the problem. The more important question is why bacteria were able to remain there in the first place. Healthy digestion depends on the coordinated movement of food through the tract, and many things influence whether bacteria stay where they belong.

Not every positive test requires treatment, and not every negative test rules SIBO out completely.
